Acknowledgement of Program Eligibility

The Home Delivery Service is available to anyone within the city limits of Sun Prairie who is unable to get to a library because of age, disability, or illness. To be eligible for Home Delivery Service individuals must meet the following criteria. 
I live within the City of Sun Prairie limits.
I am unable to get to the library because of age, illness, or disability.

Retain Information

May library staff retain information about titles you have borrowed using your library card?  (This will help prevent staff from choosing items you have already borrowed.) 

Library staff is prohibited from disclosing information contained in library records, except as outlined in the Sun Prairie Public Library Confidentiality Policy.
